Completed applications should be mailed or faxed to Dawn Matchett, Director of Intake and Resident Placement.

Hearth housing is available to low income seniors age 55 and up.

  The Check List lists everything you need to submit with the application.
   
• Hearth Housing Application
  Hearth's Housing Application is for individuals 55 years and older interested in living in Hearth's supported housing sites or Ruggles Affordable Assisted Living Community.
   
• Physician Summary Form
  Individuals applying for Ruggles Affordable Assisted Living Community also need to submit the Physician Summary Form.
