Hearth housing is available to low income seniors age 55 and up.

  The Check List lists everything you need to submit with the application.


• Hearth Housing Application
  Hearth's Housing Application is for individuals 50 years and older interested in living in Hearth's supported housing sites or Ruggles Affordable Assisted Living Community.


• Physician Summary Form
  Individuals applying for Ruggles Affordable Assisted Living Community need to also submit the Physician Summary Form.
